After a very positive response from our PPG meeting and results from our survey, we have decided to continue with our telephone triage appointment system until further notice. We continue to welcome your comments or suggestions regarding this system.


diaryHow to register

Registering at our practice could not be easier, just collect and complete a registration form (available from  reception).

You will also need to provide proof of your address and, if you were not born in the UK, some form of  photographic ID.

We will also ask you to complete a short medical questionnaire, this will allow us to record some helpful medical background information about you.

In order for us to register you quickly and efficiently please complete the resistration form and questionnaire clearly and accurately, remembering to include your previous GP details and place of birth. These details help us to trace your medical records.

Although you will be registered with us within around 7 days, it may be several weeks before we obtain your medical records.  Your new medical card will be sent out to you automatically.
